With the hoped for return to school and resumption of extracurricular activities, MDI High School has asked us to post the following information so students and families can be prepared and complete the necessary paperwork and physicals for those needing them prior to the start of school!

Due to the circumstances brought about by the Covid-19 healthcare crisis, the MDIHS administration and the athletic department have agreed to a revision of the extracurricular policy regarding physicals for this coming school year. MDIHS will follow the guidelines outlined by the Maine Principals Association Interscholastic Management Committee as follows:  “...if a student has previously participated in an extracurricular activity, they may continue to do so as long as they do not have any pre-existing medical conditions and provide a Health History Update Form completed by a parent/guardian and reviewed by a school healthcare professional prior to participation.”

NO student may begin preseason practice until the following forms are completed online:

 

  • EXTRACURRICULAR PERMISSION FORM & INSURANCE INFORMATION - these forms need to be completed online at www.rankonesport.com. More information will be given at the extracurricular meeting [date/time TBA]. Proof of insurance needs to be uploaded to this website, and the date of the most recent tetanus booster.

  • PHYSICAL EXAMINATION AND HEALTH HISTORY UPDATE FORMS - go to the nurse. A HEALTH HISTORY UPDATE FORM is available here. Doctors may document their exams in any manner for the school record OR use the back of the Pre-Participation History Form. Private exams are encouraged and should be scheduled as soon as possible.

 

 

Who needs a physical?    ALL students who are new to extracurriculars and ALL transfer students.
How can my 9th grade or transfer student receive a free physical on August 13th, August 14th or August 17th?

After receiving this information, call 288-5011 extension 3300 or 3352 between the hours of 9 a.m.-3 p.m. to schedule an appointment. Exams are held in the school clinic. The “Pre-Participation History Form”, questions 1-49 MUST be completed and signed by a parent/guardian before any student will receive a physical. [Appointments will be approximately 30 minutes to allow for cleaning]. On the day of your physical please remain in your car and call 288-5011 ext 3305 to announce your arrival in the parking lot by the main office. The school nurse will let you know when to enter the building through the middle set of doors. Please send only the student into the building who will be asked to sanitize their hands and to wear a face mask.

ImPACT Testing is mandatory for all freshmen and juniors, as well as any new students and seniors who have not had testing in the last 2 years.

Testing must be completed before ANY FALL EXTRACURRICULAR or the Teams will be assigned a test time at their 1st practice. If a student is unable to get tested on the date offered, it will be up to the student to contact the Athletic Trainer to make an appointment for ImPACT testing. Testing must be completed prior to any extracurricular participation.

 

Students doing WINTER or SPRING EXTRACURRICULARS WILL BE OFFERED OTHER DATES FOR ImPACT TESTING AND FREE PHYSICALS LATER IN THE SCHOOL YEAR.

What can I do with completed private physical information during the summer?

Forms should NOT be handed to the Coach! If you are asking a doctor's office to send forms, you should confirm with the school that the forms have been received by calling the school at (207) 288-5011 extension 3300.
